Heparin

Caution
The 10 units/mL flush and 10,000 units/mL vials look alike and mix-ups have caused fatal overdoses; heparin-induced thrombocytopenia can occur.
Storage
Room temperature; store the flush strength and the high-concentration vials apart.

Common side effects

  • Bleeding and easy bruising
  • Low platelet count (HIT)
  • Injection-site irritation
  • Elevated liver enzymes

Key interactions

  • Additive bleeding risk with NSAIDs, aspirin, and other anticoagulants
  • Reversed by protamine sulfate

Counseling points

  • Report unusual bruising, black stools, or pink urine
  • Monitored with aPTT, not INR
  • Use a soft toothbrush and an electric razor
