Common side effects

  • Bleeding or easy bruising
  • Nosebleeds
  • Blood in urine or stool
  • Anemia
  • Prolonged bleeding from cuts

Key interactions & cautions

  • NSAIDs and aspirin raise bleeding risk
  • Avoid other blood thinners unless directed
  • Strong CYP3A4 inhibitors raise levels (ketoconazole)
  • St. John's wort may lower its effect

Counseling points

  • Take 15 mg and 20 mg doses with food
  • Do not stop suddenly without prescriber advice
