Pharmacy math for the PTCE: what is actually tested in 2026
Calculations sit mainly in Order Entry and Processing, where three of the four knowledge areas are flagged as calculation-based, and they appear inside other domains too. Alligation was removed with the January 2026 outline, so the tic-tac-toe grid is no longer worth your practice time.

Math frightens more candidates than any other part of this exam, and the fear is mostly misallocated. The arithmetic itself is middle-school level. What makes questions hard is unit handling and reading the sig correctly, and both improve with repetition rather than with cleverness.
What is tested
- Days supply, from tablets, liquids, drops, inhalers, and insulin pens
- Percent strength and ratio strength, and converting between them and mg/mL
- Dilutions using C1V1 equals C2V2, and how much diluent to add
- IV flow rates in mL per hour and drop rates in drops per minute
- Milliequivalents, and reading a vial labeled in mEq/mL
- Body surface area and weight-based dosing, including mg/kg per day split into doses
- Household and metric conversions, pounds to kilograms, and temperature
- Business math: markup, discount, and inventory turnover
Notice how much of that list is one skill wearing different clothes. Days supply, IV rate, and mg/kg dosing are all the same operation: find the amount used per unit of time, then divide the total by it. Seeing the shared operation collapses three chapters of a review book into a single habit you can apply anywhere.
What came off in 2026
Alligation was removed with the outline effective January 6, 2026, along with the rest of the compounding content: not one of the 24 knowledge areas mentions compounding, sterile preparation, USP chapters, or alligation. Least weighable quantity and percent error on a balance are compounding-accuracy calculations, so they have no knowledge area to sit under either, though PTCB does not list removals item by item. The habit that prevents most errors
Carry units through every line and let them cancel. If your answer comes out in milligrams when the question asked for milliliters, the units told you before the arithmetic did. Round only at the final step, because rounding partway through compounds the error and turns a right method into a wrong answer.
The second habit is a sanity check on magnitude. Kilograms are always a smaller number than pounds. A pediatric dose is always smaller than an adult one. Days supply for a 30-tablet bottle taken twice daily cannot be 30. A quick plausibility check catches the decimal-place mistakes that cause most of the damage in real pharmacies and most of the lost points on an exam.
Practicing it
